Submission Checklist:

• One-page fact sheet (Microsoft Word format)
• Expanded Executive Summary (Microsoft Word format)
• $195 submission fee

Payment Instructions:

We accept Visa, MasterCard, and American Express through our online submission system. Payment can be made by check to the address below. The institute will then issue a pre-paid code for the submission system.
